What steps have you taken to overcome the stress of this event? (Remember to protect patient privacy). Explain the importance of debriefings. Are they used in your workplace? If a debriefing would happen who would be involved, what would be discussed, who would lead the debriefing, where would it take place? Are debriefings possible at your workplace and would they help nurses deal with stress? ( I posted below my experience with me that affects my practice) Several years ago, I was caring for a 55-year-old Hispanic patient that was being admitted for diabetic ketoacidosis. I worked in a unit called a clinical transitions unit (CTU). We handled patients that were being admitted but did not have a bed in the unit so basically a holding area. The unit managed all different types of patients such as cerebrovascular accident (CVA), angina, and or oncology patients. I recall this being a weekend, Patient Hernandez was admitted for a glucose of 900. I had received the orders to initiate am insulin drip and to give 50 units of aspart, per policy nurses must read back the telephone orders to the physician. DR X, called the order to me over the telephone. I read the order and verified this with the charge nurse. Per hospital policy this medication is a high-risk medication, this must always be verified at the bedside along with the glucose. I recalled thinking that this did not feel right. Patient Hernandez received an assigned bed in the intermediate med surge unit (IMIS). A report was called to the accepting RN and the patient was transferred stable to the unit. Maybe two-three weeks later my supervisor called me into the office questioning that specific patient. I then learned that the patient became unresponsive and the glucose had dropped to 20. After, the incident the team began looking at possible causes, and the 50 units of aspart that was given should have been 15. This incident was reviewed by the ethics and process improvement committee, I had to attend the meeting. They questioned the order and why had I given the insulin. The ordering physician should have attended but did not. This was meeting was stressful and intimidating. I learned a valuable lesson as a nurse, only because you receive an order that does not make it right. As a nurse, it is my responsibility to question orders. The debriefing that I encountered was a negative experience. My supervisor who sat next to me at the meeting, turned and questioned me before it began saying, “I have not bee a floor nurse in over 20 years and I know not to give it.” At this moment, I realize I had no support and was on my own.
